I just had a baby. How do I add them to my coverage?

To enroll a new dependent, please complete the Enrollment/Change form and submit the form to the Trust Fund Office along with the hospital copy of the birth certificate (if you do not already have your dependent’s certified birth certificate) within 30 days. Hospital certificates are valid for 90-days from the date of birth; therefore, please make sure to provide the Trust Fund Office with the certified copy of the birth certificate as soon as possible to prevent any disruptions in your newborn’s coverage.
